APPLICATION FOR MEMBERSHIP (And, Request for Replacement State Sticker and/or Pin)
Membership processing options: 1 – State 600 Bowling Club membership - $5.00 one-time, life-time fee, effective June 1, 2021. (National 600 Bowling Club membership already received). 2 – Obtain National 600 Bowling Club membership first. When membership # received, include national # on State form when applying for State membership (NOTE – EACH membership is separate and BOTH memberships are required to participate in the annual WI State 600 Bowling Club tournament). 3 – Send separate application for State membership at the same time National membership is requested, with National # “applied for.” When new member’s National # is received, send/email # to State office for membership file update. 4 – Send both applications to State office for processing (include two separate checks, one check for National - $15.00/$20.00, one check for State - $5.00). 5 – Remember, only National 600 Bowling Club ISSUES a membership CARD; proof of WI State 600 Club membership is by a STICKER placed on the reverse side of the national membership card. 6 – Request a WI State replacement sticker / pin.
